Position Summary
The Case Manager is responsible for ensuring all service needs of a consumer are met by providing
linking, coordinating, advocacy, negotiating and referral services. The Case Manager is responsible
for the assurance of a Person Centered Plan as well as the monitoring of the established goals and
the implementation of various treatment plans.

Specific Responsibilities

• Develops an individualized plan of service based on the comprehensive assessment which
  includes goals established by all appropriate disciplines
• Acts as the Chairperson of the multidisciplinary team
• Conducts self audits as well as peer audits
• Must be knowledgeable in the Mental Health Code and Medicaid guidelines as well as billing
  protocols and procedures
• Coordinates and completes all necessary referrals on behalf of client
• Monitor all services provided on behalf of client
• Monitor satisfaction with services
• Provide resource development in an attempt to reduce the amount of service gaps
• Develop and maintain written records necessary for professional practice as defined in the
  procedural manual
• Provide outreach services to individuals and their families in their respective locales and/or
  homes as appropriate to their unit’s function
• Provide community education services as assigned
• Participate in clinical supervision, team meetings and case consultations
• Provide services in the field as necessary
• Obtain all prior authorizations and applicable coordination of care
• Follow PsyGenics, Inc. professional code of conduct

This is a full-time position – Monday – Friday from 8:30AM – 5:00PM. The position is not remote.

The Supports Coordinator Assistant is responsible for ensuring all service needs of a consumer are met by providing linking, coordinating, advocacy, negotiating and referral services under the supervision/guidance of a Supports Coordinator.  The Supports Coordinator Assistant is responsible for the assurance of a Person Centered Plan as well as the monitoring of the established goals and the implementation of various treatment plans in conjunction with the multi-disciplinary team and under the guidance of the Supports Coordinator. The Supports Coordinator Assistant will act as an advocate for the consumers and community served and ensure delivery of services.

Specific Responsibilities

• Develops an individualized plan of service based on the comprehensive assessment which includes goals established by all appropriate disciplines
• Conducts self audits as well as peer audits
• Must be knowledgeable in the Mental Health Code and Medicaid guidelines as well as billing protocols and procedures
• Ensure delivery and monitoring of services as prescribed in the Person Centered Plan
• Maintain PsyGenics’ expected productivity level
• Completes and secures required signatures
• Assists Supports Coordinator in the delivery of referrals, services and implementation of the Person-Centered Plan
• Coordinates and completes all necessary referrals on behalf of client
• Monitor all services provided on behalf of client
• Provide resource development in an attempt to reduce the amount of service gaps
• Develop and maintain written records necessary for professional practice as defined in the procedural manual

Qualifications

Must possess and retain a current license as a Registered Occupational Therapist in the State of Michigan.  At least one year relevant experience with persons with a developmental disability is preferred, however will consider new graduates. Position Summary

The Occupational therapist, in compliance with agency policy and procedures, is responsible for providing direct services to clients diagnosed with a Developmental Disability. Must also complete evaluations, recommendations, as well as develop treatment goals and service plans.  Therapist is also responsible for periodic reviews and all documentation pertinent to discipline as well as requested by the Supports Coordinator.  Also must provide community education, consultation with other interdisciplinary team members and respond to consumer service requests. Regular working hours are to be determined based on the needs of the consumers and of the program and may include some evenings and occasional weekends.  Typical business hours are 8:30am-5:00pm.

Specific Responsibilities

• Conduct thorough occupational therapy assessments/evaluations in compliance with agency and Medicaid Standards
• Design client treatment plans that are based on clients’ needs and the least restrictive treatment services.
• Develop measurable and objective goals that directly reflect the PCP assessment.
• Function as a member of the I-team to determine the least restrictive type and scope of treatment.
• Provide consultation and training to other staff as requested.
• Provide community education, consultation with other interdisciplinary team members
• Respond to consumer service requests
• Develop and maintain written records necessary for professional practice as defined in the procedural manual.
• Train and supervise students, volunteers as directed.
• Complete necessary equipment and home modification requests in accordance with program guidelines.
• Must complete 24 hours of program and/or position specific training on an annual basis and must maintain active crendentials of QIDP, CMHP, QMHP.
• Productivity of 65% or greater is expected
• Fieldwork is required
• Provide treatment/interventions based on consumer needs such as in clinic, in home or via telemedicine when deemed appropriate by DWIHN

Position Summary

The Link To Wellness nurse ensures integrated health care by collaboration, coordination and a comprehensive approach to healthcare for individuals diagnosed with a developmental disability. They are responsible for the development and monitoring of objective and measurable health goals, completing prescribed visits and acting as a member of a multi-disciplinary team.

Specific Responsibilities

• Complete Initial and Annual Comprehensive Health Assessments
• Develop measurable and objective goals based in Person-Centered Planning
• Collaborate with Primary Care Physician, Psychiatrist and all medical professions to coordinate care
• Monitor written goals and objectives and amend when applicable
• Complete face-to-face encounters
• Provide consumer and family education, training and ongoing supervision
• Document progress or lack of within the timeframes directed
• Coordinate with Supports Coordinator as well as all team members regarding consumer care, changes, updates, etc.
• Complete charting, logs, graphing, etc. of consumers’ progress
• Conduct/complete audits
• Excellent communication skills both written and verbal

In lieu of in-house supervision, Limited Licensed professionals may also choose to provide documentation of ongoing outside supervision on a quarterly basis until such time as they receive full licensure.

Qualifications

Must possess a master’s degree in psychology from an accredited college or university.  Applicant must also have a TLLP*, LLP* or LP in Psychology with the State of Michigan in good standing.  Must have police and DHS clearance.  Person must possess experience with persons with developmental disabilities and/or experience with persons with a Traumatic Brain Injury. Must meet criteria for appropriate credentialing as a Children’s Mental Health Professional, Qualified Intellectual Disabilities Professional and a Qualified Mental Health Professional. Position Summary

The Psychologist is responsible for psychological evaluations of consumers including administration and the interpretation of psychometric instruments while providing direct services to agency consumers. The psychologist is also responsible for the development of measurable and objective treatment goals and plans in accordance with Person Centered Planning, thorough assessments, individual counseling, parent training, family training, behavior modifications plans, and crisis intervention.  The Psychologist must also participate as an active member of the Behavior Treatment Committee and maintain all requirements of said committee.

Specific Responsibilities

• Administer psychological evaluations of consumers, interpret and create written report
• Provide consultation to staff regarding consumer, treatment plan, environmental considerations as well all applicable considerations
• Provide direct therapeutic counseling and intervention to individuals and families
• Conduct staff in-services specific to consumer served to assigned direct care staff
• Monitor implementation of treatment plan by I-team members, consumer and family
• Obtain, analyze and evaluate significant data to develop service plans and construct programs utilizing a combination of treatment modalities and skills
• Provide support services to individuals and families
• Develop and maintain written records necessary for professional practice as defined in the policy and procedural manual.
• Represent PsyGenics Inc. in the community
• Conduct community outreach and education

• Act as an advocate on behalf of consumer
• Complete guardianship process and testify in court
• Productivity of 60% or greater is expected
• Conduct self-audits as well as peer audits
• Perform similar or related duties as assigned

*Limited Licensed professionals may also function in this role with the understanding that they must maintain ongoing supervision until such time as they receive full licensure.  If appropriately licensed staff are available and able, PsyGenics will assist with supervision. Qualifications

Must possess a master’s degree in social or behavioral health, be fully licensed* in the State of Michigan in good standing as well as meet the criteria for a Qualified Intellectual Disabilities Professional and Qualified Mental Health Professional. Person must have police and DHS clearance. Must have a minimum of one-year post graduate experience working with children and adults with Developmental Disabilities, Severe Mental Illness or persons with a Traumatic Brain Injury. Position Summary

The Assessment Specialist is responsible for completion of annual clinical assessments and intakes for new cases. Upon assessment, initial referrals are made to all appropriate internal programs. Demonstrated ability to complete all intake documentation thoroughly including but not limited to consents for treatment, releases of information, biopsychosocial assessments and preliminary plans. Timely and skillful handoff to the Case Manager is required to ensure development of a Person-Centered Plan. As a secondary function, this position provides case management coverage to support during a period of staff leave.

Specific Responsibilities

• Provides an overview of PsyGenics and the services offered internally and externally to new members
• Maintains current information regarding both internal and external services and resources
• Explains and obtains consents for treatment, financial determination and other intake documents to members and families
• Completes a thorough biopsychosocial assessment based on sound clinical rationale
• Assists non-master’s level staff with completion of annual biopsychosocial assessments
• Develops a preliminary treatment plan based on the needs identified in the biopsychosocial assessment
• Refers and schedules new intakes with internal services including but not limited to Case Management, Psychiatrist, Link to Wellness, and other appropriate disciplines
• Completes an intended beneficiary survey with new members after the intake process
• Provides resources as appropriate
• Completes all documentation in the Electronic Medical Record system within 24 hours
• Maintains updated calendar in the Electronic Medical Record daily
• Maintains at least 50% productivity (i.e., face to face contact)
• Collaborates with Clinical Supervisor and other team members
• Must be knowledgeable in the Mental Health Code and Medicaid guidelines as well as billing protocols and procedures
• Participates in clinical supervision, team meetings and case consultation
• As appropriate, obtain all prior authorizations and applicable coordination of care
• Provides temporary gap coverage in the absence of primary case holder including completion of treatment planning, coordination of services, and referrals.

Position Summary

The Quality Specialist is responsible for monitoring, evaluating and educating the PsyGenics team to meet contract standards and ensure high quality services are delivered to our members by trained and competent staff. The position also requires providing education and coaching to PsyGenics team members regarding performance expectations for service provision and delivery, safety standards and other training related to best practice.

Specific Responsibilities

• Perform and document quarterly and annual audits as directed by funding source to ensure ongoing conformance to required standards and identification of areas for performance improvement.
  • Includes but is not limited to chart reviews, Medicaid claims, Adverse Benefit Determination audits, program specific reviews, electronic data validation reviews, etc.
• Ensure readiness for regular site reviews by funder including but not limited to Customer Service, Recipient Rights, and Quality teams.
• Complete and submit corrective action plans if required.
• Coordinate with PsyGenics Medical Director to ensure completion of Physician Peer Reviews monthly.
• Oversee incident report module, review incident reports, and report on incidents monthly.
• Review critical and sentinel events and send to case managers and supervisors at least monthly
• Assist with root cause analysis completion
• Track and trend critical events, sentinel events and root cause analyses
• Conduct the Safety Committee Meeting monthly
• Monitor Performance Indicators 2, 3, 4, and 10 and coordinate with leadership and funder as needed for follow up
• Train staff on quality assurance, health and safety, and HIPAA/Privacy standards as needed
• Conduct analysis and develop quality-related reports for leadership including data analysis on all auditing, monitoring, and related quality activities
• Participate in monthly PsyGenics Quality Improvement Committee
• Attend meetings with our funder including but not limited to monthly Quality Operations Meeting, Performance Indicator workgroup, Customer Service Meeting
• Assists in ongoing accreditation activities for CARF compliance
